This document provides practical guidelines for schools to ensure that transition from primary to secondary school is as 'seamless' and constructive as possible for all children.
The guidelines have been updated to include pupils with special educational needs and disabilities and other vulnerable pupils.
It is essential that schools should make sure that pupil progression and curriculum continuity are in place for all pupils as they move across the phases.
In this way they will support both high standards of achievement and pupils' enjoyment of their time in school.
In order to promote smooth transition, there are huge implications for early consideration by both primary and secondary schools in the Autumn Term.

We have included a 'Transition Diary' which you may find useful in September.
We are sending all larger schools two copies: one for the SENCO and one for the Y6 team leader.
